Why Is My Cholesterol High If I Eat Healthy?

If your cholesterol is high despite salads and daily walks, your liver and blood sugar may be steering the numbers far more than your fork is.

You eat the salads. You skip the drive-through, rarely touch dessert, and walk most days. Then your doctor says your cholesterol is high, and it makes no sense. Here's the part nobody explains at that appointment: most of your cholesterol is made inside your body, not eaten, and your liver runs that whole operation. When the liver is overloaded with fat it's quietly storing, the numbers can climb no matter how clean your plate looks.
So if you left that visit thinking I'm doing everything right, how is this possible, you're asking exactly the right question. The answer usually has very little to do with the eggs you gave up.
Isn't high cholesterol caused by what I eat?
That's the story most of us grew up on: eat cholesterol, get high cholesterol. It isn't wrong so much as it's a thin slice of a much bigger picture.
Your liver makes roughly 75 to 80 percent of the cholesterol in your blood, and it has good reason to. Cholesterol builds your hormones, holds your cell membranes together, lets your skin turn sunlight into vitamin D, and supports your brain. It's also the raw material your liver uses to make bile for digestion. None of that is the enemy. The real question isn't whether cholesterol exists in your body. How does my liver actually control cholesterol?
Picture your liver as the dispatch center for cholesterol. It produces what you need, packages it for transport, pulls the excess back out of circulation, and converts a good portion into bile to send off through digestion.
When that center runs smoothly, the whole system stays balanced. Bog it down with stored fat and the dispatching gets sloppy. Cholesterol that should have been cleared or converted lingers instead. This is how a woman eating a genuinely good diet still ends up with high LDL or high triglycerides on her labs. The food isn't the bottleneck. The factory is.
What does fatty liver have to do with my numbers?
Fatty liver, the kind not caused by alcohol, simply means fat has built up inside liver cells where it doesn't belong. As that fat accumulates, the liver gets less efficient at handling both fats and sugars, and the lab work tends to drift in a recognizable direction:
- Higher LDL cholesterol
- High triglycerides
- Lower HDL, the protective kind
- Rising blood sugar or early insulin resistance
- Weight settling around the middle
What makes this so easy to miss is that fatty liver is quiet. It rarely hurts and waves no obvious flag, just a slow creep of fatigue, brain fog, and numbers that won't budge. The cholesterol shows up on paper long before anyone thinks to look at the liver.
Why doesn't eating even cleaner fix it?
Here's the frustrating part. The instinct is to double down: cut the eggs, drop the red meat, slash calories, eat almost no fat at all. And the cholesterol barely moves.
It barely moves because diet is only one lever among many. Your numbers also answer to blood sugar, insulin, hormones, sleep, stress, movement, and the genes you were handed. When insulin resistance is the thing pushing your triglycerides up, eating less fat doesn't touch the cause. You can run a perfect diet and still get a discouraging result, because the lever you're pulling isn't connected to the problem.

Is insulin resistance behind this?
Often, yes. It's one of the biggest hidden players in stubborn cholesterol. Insulin resistance means your cells stop responding well to insulin, so your body pumps out more of it to compensate. Over time that extra insulin changes how your liver handles fats and sugars, and a familiar signature appears: triglycerides up, HDL down, weight gathering at the waist.
Insulin resistance and fatty liver travel together so reliably that supporting one tends to help the other. That's the hopeful flip side. Steady the blood sugar, ease the stored liver fat, and several markers can move in the right direction at once instead of you fighting each one alone.
What can I actually do about it?
Shift your attention off the cholesterol number and onto the systems producing it. When blood sugar steadies and the liver gets some breathing room, the labs tend to follow. A few things that genuinely help:
- Build meals around protein and eat on a steady rhythm rather than grazing or crash-dieting
- Add soluble fiber from oats, beans, chia, and apples, which helps your body carry cholesterol out through bile
- Take a 10-minute walk after meals to blunt the blood-sugar spike
- Protect your sleep and find a real outlet for stress, since both quietly raise blood sugar
If high cholesterol is showing up alongside belly weight, fatigue, brain fog, cravings, or right-sided rib discomfort, it's worth asking your healthcare provider to look at your liver and metabolic health, not just your lipids. So often this is less a cholesterol problem and more a signal, and a signal is something you can work with.
